Definition Lichenoid reactions are a heterogeneous group of lesions of the oral mucosa that show clinical and histopathological similarities to lichen planus, but have a different course. Etiology Hypersensitivity to dental restorative materials, amalgam, composite resins and dental plaque accumulation are the most common causative factors. Lichenoid drug eruptions (LDE) are usually pruritic. The onset may be within weeks of drug initiation, but LDE is atypical in that it can take up to 12 months to evolve. A latent period of 3 years has been reported with penicillamine. LDE from ACE inhibitors typically takes 3 to 6 months to appear. Lichenoid drug reactions frequently occur months to years after the initiation of drug therapy.
